چکیده

A multicenter prospective study was performed in 49 patients with 77 regions of cerebral infarction. Each patient was evaluated in the acute (0-5 days) and subacute (6-17 days) phases by (1) clinical neurologic examination, (2) CT scans, and (3) Nisopropyl-p-1-iodoamphetamine ('IMP) single-photon emission CT (SPECT) scans. The abilities of the scans to (1) detect a lesion and (2) predict the clinical outcome were assessed. For lesion detection, 1MP-SPECT was superior to CT in the first 2 days, but the scans were equally effective 3-5 days after onset. In the subacute phase, IV contrastenhanced CT was superior to IMP-SPECT and unenhanced CT. The clinical outcome was only mildly correlated with the results of the acute and subacute 1MP-SPECT and the acute CT scans. Reduction in lesion size on the subacute scans did not correlate with clinical improvement. We conclude that the parameters measured by CT and 1MP-SPECT in patients with acute cerebral infarction cannot reliably be used to predict clinical outcome. 1 contamination of 1MP and the use of low-energy collimators may have decreased lesion detectability.

Enzymatic and Immunogenic Changes of Myocardial Infarction

Diagnosis of myocardial infarction is very important in a patient with chest pain. Chest pain is not always present or it may be from other causes rather than myocardial infarction. Q wave is present in 30] of infarction. ST,T wave changes are present in all myocardial infarctions, but these changes also are present in myocardial ischernia.  For these reasons diagnosis of myocardial infarction...

Fungal infections of the CNS - Section 1

While fungal infections of the CNS are relatively rare, they have become more common with the increasing number of individuals who are immunocompromised due to HIV/AIDS, immunosuppressive therapies, invasive diagnosis and treatment methods, and organ transplants. CNS fungal infections present many diagnostic and therapeutic challenges and are associated with a high mortality rate. Cryptococcal ...

Inhibition of Angiotensin-Converting Enzyme Reduces Cerebral Infarction Size in Experimental-Induced Focal Cerebral Ischemia in the Rat

Background: The role of Renin Angiotensin System (RAS) in ischemic/reperfusion (I/R) injuries is not fully elucidated. Furthermore, it is not clear whether inhibition of RAS by Angiotensin-Converting Enzyme (ACE) inhibitors has beneficial effects in terms of protecting the brain from I/R injuries. In this study enalapril is used as an ACE inhibitor to evaluate the role of RAS in I/R injuries in...

The Willis lecture: transient ischemic attacks, scientific method, and new realities.

First described by Thomas Willis in 1679 and defined in the 1950s, transient ischemic attack is universally agreed to be an episode of focal neurologic deficit in a vascular distribution, sudden in onset and resolving without residual deficit in less than or equal to 24 hours. Transient ischemic attack is diagnosed by characteristic history and absence of residua on neurologic examination. Afte...

THE EFFICACY OF DECOMPRESSIVE CRANIECTOMY IN TREATMENT OF PATIENTS WITH MASSIVE HEMISPHERIC CEREBRAL INFARCTION

Massive cerebral infarction is often accompanied by early death, secondary to brain edema and trans-tentorial herniation. Several reports indicate beneficial effects of decompressive craniectomy in this situation, but the efficacy of this procedure is still a matter of debate. A n experimental study in a period of3 years was done on2 3 patients with brain edema due to massive cerebral infa...

QT-Dispersion as a potential marker in prognosis of acute myocardial infarction

Introduction: QT dispersion in a 12 lead ECG represents the heterogeneity of ventricular repolarization. The prolongation of QT-interval dispersion increases the risk of coronary heart disease, ventricular arrhythmia, and sudden cardiac death in patients with myocardial infarction. We aimed to investigate the prognostic value of QT-dispersion in patients with acute myocardial infarction. Meth...
